DEPARTAMENTO ADMINISTRATIVO DISTRITAL DE SALUD-DADIS<\/b>, le recuerda que acorde a la Resoluci\u00f3n 3100 de 2019 usted deber\u00e1 realizar el proceso de autoevaluaci\u00f3n de los servicios antes del d\u00eda\u00a0<b>de\u00a0vencimiento<\/b>,<b>\u00a0<\/b>fecha en la cual se le vence su habilitaci\u00f3n en el Registro Especial de Prestadores de Servicios de Salud, este proceso lo deber Adicionalmente, si se trata de una instituci\u00f3n prestadora de servicios de salud, requerir\u00e1 visita de reactivaci\u00f3n por parte de la entidad Departamental o Distrital, seg\u00fan corresponda, cuyo objeto consistir\u00e1 en la verificaci\u00f3n de las condiciones de habilitaci\u00f3n establecidas y en la decisi\u00f3n sobre la inscripci\u00f3n.<\/p><p>\u00a0<\/p><p>Para los casos en que el prestador no haya autoevaluado uno o varios de los servicios habilitados, con su correspondiente declaraci\u00f3n en el REPS, proceder\u00e1 la inactivaci\u00f3n del servicio o servicios no autoevaluados en dicho registro. Adicionalmente, si se trata de servicios de alta complejidad, urgencias, hospitalizaci\u00f3n obst\u00e9trica, transporte asistencial u oncol\u00f3gicos, requerir\u00e1 visita de reactivaci\u00f3n por parte de la entidad departamental o distrital, seg\u00fan corresponda, cuyo objeto consistir\u00e1 en la verificaci\u00f3n de las condiciones de habilitaci\u00f3n y en la decisi\u00f3n sobre la inscripci\u00f3n.<\/p><p>\u00a0<\/p><p>Para mayor informaci\u00f3n y ante cualquier duda favor acercarse a las Oficinas de Vigilancia y Control del DADIS ubicada en el Centro, barrio Getseman\u00ed, Calle Larga, Carrera 10B No 25-01, Casa F\u00e1tima Piso 2.<\/p><p>\u00a0<\/p><p>NOTA: Para realizar el procedimiento de autoevaluaci\u00f3n deber\u00e1 ingresar usuario y contrase\u00f1a.
